USM Door Access Request Process

Summary

This is an outline to assist USM Faculty and Staff for card access or removal request.

Body

USM requires all patrons (except residential students) to submit a completed USM access request form.  This form must be signed by a USM authorized approver who will in turn submit the form to card.services@maine.edu for processing.

 

USM Door Access Request Process:

  • Requests for card access for employees, students and temporary access must be submitted by completing this request form; Card Access Request
    • Please include the requested campus location in the subject line (Gorham/LAC/Portland).
  • New employees who require card access on their campus card must first visit the USM Card office to obtain their card and have their picture taken.
  • The completed form should be forwarded to the appropriate leadership located here; USM Leadership Designee, for approval and signature.
  • Designee will email completed request form to; card.services@maine.edu.
  • Cards will typically be activated within one to two business days of receiving the card access request form.
    • Online (tap) door access will be added remotely and not require patron to visit the card office
    • Offline (swipe) door access will require patron to visit the card office to have their card encoded
  • Staff and faculty will receive an email once access is ready for use.
  • Students requiring access to buildings or labs must be registered for the current semester.
  • Requests for changes to existing card access must be made by completing a new request  form and following the procedure noted above.
