Student Study Away Request Form

INSTRUCTIONS:

This form must be completed by students and approved by their home university before enrolling in courses away from their home university, whether inside or outside the University of Maine System.  The student must complete this form each semester they are taking courses that are "away" and a separate form must be completed for each "away" institution if you plan on attending at more than one institution in a term.

1. Meet with your academic advisor/department to determine appropriate courses to take at the "away" university.
2. Submit this completed form to your academic advisor/department.
3. After completing your courses at the "away" university, be sure to request that an official transcript is sent to your home university so that qualifying credits can be awarded. 

Note: Transcripts are not required for courses completed at another UMS campus.
