Description
Use this form to request an exemption or accessibility accommodation from multi-factor authentication (MFA) for a University of Maine System (UMS) account. All exemption requests are reviewed by the Information Security Office. Most exemptions are temporary.
Required Information
The following information is required when submitting this request:
- Your full name and @maine.edu email address
- A description of the specific barrier preventing you from completing standard MFA
- The type of exemption you are requesting (accessibility accommodation, lab/shared device, or other)
- Any alternatives you have already considered or attempted
- For lab computers, instructor stations, or technical equipment: the device name, location, and a description of the physical limitation (e.g., no USB or Bluetooth access)
