MFA Exemption or Accessibility Accommodation Request

Description

Use this form to request an exemption from multifactor authentication (MFA) for a University of Maine System (UMS) account. All exemption requests are reviewed by the Information Security Office. Most exemptions are temporary.

Before requesting an exemption, review the accessible MFA options already available at UMS. See MFA Methods That Support Accessibility Needs and MFA Authentication Methods.

ℹ️ INFO: Accessibility accommodation requests are submitted by your accommodation office, not by you. Employees and guests should contact the ADA Coordinator in the Office of Equal Opportunity and Title IX Services. Students should contact their campus Student Services office. Visit UMS Disability & Accessibility Services to start a request or find your campus contact.

Required Information

The following information is required when submitting this request:

  • Requestor: the person the exemption is for
  • Exemption type: accessibility accommodation, lab or instructor station or technical equipment, or other
  • Description of barrier: why standard MFA cannot be completed (exclude confidential information).
  • Alternatives already tried: which available MFA methods you have attempted
  • Requested duration: annual (subject to yearly review), or temporary with a specified end date

For lab computers, instructor stations, or technical equipment, include the device name, location, and physical limitation (for example, no USB or Bluetooth access for a security key).
